Gifford Grade School would like to encourage each family to fill out the 5 Essentials Survey.
PARENT SURVEY
The parent survey is to be completed by parents, guardians, or other caregivers with a child in grades pre-kindergarten through 12. The parent survey is available in English and Spanish and takes approximately 10 minutes to complete. If a parent has more than one (1) child and they attend the same school, that parent may elect to participate multiple times. Only one parent per household may submit a survey for a given child.
Note: Parents have the option to leave their email address so they can be sent confirmation of survey completion upon submission of the parent survey supplement. That text box can be left blank if the parent doesn’t want to provide or have an email address.
